About The Position

Family Connection Consultants will implement the Family Connections (FC) model and curriculum with fidelity and follow all program and agency policies and procedures. This role involves conducting home visits, developing service plans, connecting families to resources, and collaborating with various agencies and partners to support families in achieving their goals.

Requirements

  • Minimum of a Bachelor’s Degree in Social Work or related field.
  • At least one (1) year of relevant experience.
  • Knowledge of community resources, case management, assessment and service planning, and problem solving with complex family needs.
  • Understanding of DCS systems is preferred.
  • Ability to work under pressure, meet deadlines, and set priorities.
  • Well-organized and able to effectively manage multiple cases.
  • Ability to work independently and as part of a team.
  • Flexibility and ability to adapt to changing working conditions.
  • Sensitivity to cultural differences within the agency and community.
  • Fluent in Microsoft Office Applications.
  • Intermediate-level technology and software use for documentation and casework.
  • Must be able to stoop, bend, squat, sit on floor and ambulate over uneven floor surfaces including stairs and steps.
  • Must be able to visually and auditory assess and interact with clients to ensure their safety and wellbeing.
  • Requires willingness to be on-call for emergencies.
  • Requires evening and weekends work.
  • May lift up to 25 pounds.
  • Valid AZ driver’s license, reliable transportation and ability to show proof of registration/insurance.
  • Level One Fingerprint clearance or ability to obtain fingerprint clearance; must maintain clearance.
  • Must be 21 years of age or older to drive on behalf of Catholic Charities.
  • Must have or complete within 90 days of hire, training in CPR and First Aid and remain current in the certification.

Responsibilities

  • Implement the Family Connections (FC) model and curriculum with fidelity, adhering to all program and agency policies and procedures.
  • Conduct home visits at a minimum of once a week and deliver the FC Curriculum.
  • Prepare for home visits and complete documentation within one business day of completion.
  • Conduct home visits at times convenient for the family, which may include evenings and weekends.
  • Conduct comprehensive family assessments and other required assessments according to the Family Connections Model and contract guidelines.
  • Develop and implement Service Plans with family participation, ensuring UBSMART goals and interventions to support family outcomes.
  • Maintain case records and data systems in accordance with agency and program policies.
  • Initiate contact and conduct a home visit with referred families within one business day of referral.
  • Attend mandated virtual or in-person trainings, conferences, meetings, or community events, which may include out-of-state travel.
  • Connect families to resources that address their needs.
  • Maintain a positive working relationship with colleagues, administration, collaborative partners, and others within the community.
  • Participate in one hour of individual supervision and one hour of group supervision per week.
  • Make appropriate referrals to government & community agencies and sub-contractors, following established procedures and forms, and follow up on progress.
  • Report suspected child abuse or neglect to the Department of Child Safety (DCS).
  • Complete training in the Nurturing Parenting Program practice model.
  • Utilize or conduct intakes per the Nurturing Parenting model.
  • Collaborate with the parent/caregiver to gather information and complete the Family Nurturing Plan (FNP).
  • Conduct a minimum of two hour weekly in-person parenting sessions with the family and document progress.
  • Schedule a mid-point session with the family to evaluate progress towards NNP goals.
  • Close cases when targeted UBSMART goals have been addressed, the client requests closure, service provision can no longer achieve goals, or identified reasons per the Nurturing Program Manual have been met.
  • Complete documentation required for case closure.
  • Promote ongoing commitment to excellence and continual quality improvement.
  • Perform other duties as assigned.
