About The Position

The Wyoming Behavioral Institute Outpatient & Telehealth Clinic provides tele-mental health services to youth and their families across the State of Wyoming. A Certified Family Care Coordinator (FCC) serves the role as care coordinator to support access to services for children and families. FCC’s are trained and certified in Magellan’s High Fidelity Wraparound care model and are responsible for comprehensive, strength-based, and culturally responsive support to families with complex needs. FCCs work within the community settings and must be able to effectively collaborate with community members including social service, educational, and government agencies to coordinate the implementation of assessments, crisis plans, and care plans to support children and families. FCCs are responsible for providing accurate and timely communication, providing direct care to students and their families, completing reporting/data entry and documentation with accuracy, and participating in problem-solving and making recommendations to referral sources as well as the treatment team. Other duties as assigned.

Requirements

  • Must be 21 years of age
  • Must be a currently active/enrolled Certified Family Care Coordinator
  • Experience working in confidential records
  • Experience completing documentation
  • Understanding of Magellan timeline and billing processes
  • Demonstrated ability to successfully organize and enter data
  • Demonstrated ability to complete reporting
  • Demonstrated ability to organize patient files
  • Demonstrated ability to communicate effectively through teleservices
  • Demonstrated ability to meet licensing, accreditation, and certification standards
  • Must have knowledge of developmental tasks of children, adolescents, adults, and families and apply knowledge in assessment and planning of care
  • Must be able to maintain the ability to provide and support services through telehealth, including the need to speak and hear via teleconference

Nice To Haves

  • Preferred associate’s degree (or higher) in mental health/behavioral health, Social Sciences, Social Work, or related field
  • Preferred two (2) years of experience in patient care and mental and behavioral health care or equivalent
  • Expertise in using Zoom, excel, word, OneDrive, outlook email and calendar, and Magellan’s fidelity program

Responsibilities

  • Contact families within 72 hours of service authorization to initiate engagement and enrollment
  • Explain High Fidelity Wraparound services, provider choice, and family rights
  • Complete intake documentation, consent forms, and provider selection records
  • Coordinate respite service authorization when applicable
  • Complete application process within 7 days
  • Complete required assessments within 60 days of enrollment, including: Family Story, Child and Adolescent Needs and Strengths (CANS), Adverse Childhood Experiences (ACE) screening
  • Develop and maintain individualized, strength-based Plans of Care
  • Ensure youth, family, and legal guardian sign-off on all plans
  • Review and update care plans at least every 45 days
  • Facilitate Child and Family Team (CFT) meetings, including crisis and emergency meetings
  • Support families in identifying and engaging natural and formal supports
  • Ensure meetings are family-directed regarding time and location (unless safety concerns arise)
  • Coordinate services across behavioral health, medical, educational, and community systems
  • Assist families in scheduling and attending appointments
  • Ensure youth are connected to EPSDT services (e.g., primary care, dental)
  • Conduct at least two face-to-face contacts every 30 days with youth and caregivers
  • Monitor progress toward goals and adjust services as needed
  • Develop individualized safety and crisis plans within the first 60 days
  • Provide coordination and support during behavioral health crises
  • Document critical incidents (including out-of-home placements) within 24 hours
  • Enter all required documentation into the approved Electronic Health Record (EHR) system within 3 business days
  • Track key milestones such as: Out-of-home placements, Youth turning 18, Length of stay exceeding 18 months
  • Ensure full compliance with HIPAA and applicable privacy regulations
  • Maintain fidelity to the National Wraparound Initiative standards
  • Participate in fidelity monitoring using the Wraparound Fidelity Assessment System (WFI-EZ)
  • Support youth and caregivers in completing fidelity assessments
  • Participate in required training, certification, and annual recertification
  • Provide wraparound services for students and families as outlined below: 32-hour FCC (32 hours dedicated to FCC, Minimum of 22 hours are direct/billable services), 40-hour FCC (40 hours dedicated to FCC, Minimum of 30 hours are direct/billable services)
  • Maximum caseload: 15 youth
