About The Position

KVC Kentucky is seeking a Family Care Coordinator to work directly with families on a day-to-day basis while managing and coordinating client care. The position provides in-home services to children and families, with the goal of reducing risks, improving child well-being, and preserving child placement with the family. The role requires a flexible schedule, including nights and weekends as needed, travel throughout the region, and participation in an on-call rotation.

Requirements

• Bachelor’s degree in Social Work or another related field.
• Ability to provide in-home services to children and families.
• Experience working with complex and multi-need families.
• Valid driver’s license and proof of auto insurance.
• Flexible work schedule, including nights and weekends as necessary to meet families’ needs.
• Availability to travel throughout the region.
• Participation in an on-call rotation.

Responsibilities

• Make initial contact with families within 24 hours of referral to assess safety, initiate safety planning, and connect families with resources to meet immediate needs.
• Conduct team meetings and maintain communication with families and providers to monitor progress toward treatment goals and address barriers to services and goal attainment.
• Facilitate each family’s Action Plan by contacting identified providers, connecting providers with the family, maintaining ongoing communication, and coordinating care throughout services.
• Maintain frequent and consistent contact with DCBS personnel.
• Provide direct services to reduce risks, improve child well-being, and preserve child placement with the family in accordance with program guidelines.
