About The Position

The Family-Based Intensive Treatment (FIT) Support Worker is a member of a small, dedicated team, including a FIT Clinician and a Family Partner, focused on helping youth and families. Family-Based Intensive Treatment (FIT) is a new service within JRI's Community Service Agencies, providing medically necessary intensive family therapy, robust care coordination, and Family Partner engagement for eligible youth with serious emotional disturbance (SED) and their families. The role focuses on improving the youth’s mental health concerns while strengthening the family structure and support. Staff provide FIT services in home and community settings, fostering consistent, strength-based therapeutic relationships with the families served.

Requirements

  • Must be at least 21 years of age
  • Bachelor’s degree in a human service field from an accredited university with one (1) year of direct relevant experience working with youth and families who require behavior management to address mental health needs
  • Experience working with children is required
  • Able to obtain and interpret information in terms of the needs of emotionally fragile, mentally ill youth and their families
  • Understanding of the range of interventions and treatment required by this population
  • Maintains a valid driver’s license
  • Pass background checks and driving checks

Responsibilities

  • Provide a variety of community-based services to children, adolescents and their families
  • Deliver services including Skill coaching, Job Coaching, Life Skills training, activity planning, community exploration & integration, behavior management, assessment, treatment planning services, crisis management and case management services
  • Maintain positive relationships with outside agencies, families, and other involved parties
  • Serve as a member of a multi-disciplinary treatment team to find effective methods of guiding and supporting youth within the community
  • Support the clinical care of youth and their families in a home and community-based setting to promote healthy functioning
  • Transport youth as required
