The Faith Community Outreach Nurse provides community-based nursing services at approved faith community and community partner sites, including blood pressure screenings and flu vaccination clinics. Delivers public-facing workshops, in-person training, and health education presentations. Practices within the California RN scope of practice, Hoag policies, approved orders and FCN workflows, and Faith Community Nursing scope and standards of practice. Provides nursing care through compassionate presence, active listening, respectful inquiry, culturally and spiritually sensitive communication, appropriate referral, follow-up, documentation, and maintains professional boundaries. Provides assigned community-based nursing services, including community assessment, health promotion and disease prevention, approved screenings and clinics, health education, resource navigation, referral, follow-up, integrating intentional care of the spirit throughout these activities. Represents Hoag professionally and escalates concerns involving safety, scope, documentation, partner relationships, service requests, role clarity, or compliance to the FCN Manager or designated resource. The position requires local travel to approved faith and community sites, Hoag locations, senior centers, and community events; and may require evening or weekend work based on program and/or partner needs. Does not routinely include individual visits to homes, hospitals, nursing facilities, or residential care settings; however, limited visits may be assigned or approved by the FCN Manager based on program needs and within approved FCN scope and workflows. Performs other duties as assigned.
