About The Position

The School of Pharmacy is seeking a Faculty member in Pharmacy Practice with a focus on Ambulatory Care. This role involves coordinating and delivering instruction in pharmacotherapeutics and pharmacy practice, particularly in ambulatory care and chronic disease management, for both traditional and online pathways. The faculty member will also maintain an active ambulatory care clinical practice, provide direct patient care, and serve as a preceptor for Doctor of Pharmacy students during their Introductory and Advanced Pharmacy Practice Experiences. The position emphasizes fostering clinical reasoning, patient-centered care, and professional competencies.

Requirements

  • Doctor of Pharmacy (Pharm.D.) degree from an ACPE-accredited program.
  • Completion of an ASHP-accredited PGY1 Pharmacy Residency.
  • Eligibility for pharmacist licensure in New York State.
  • Demonstrated experience in ambulatory care practice, including chronic disease state management, medication optimization, patient education, and collaborative care.
  • Strong interpersonal, communication, and organizational skills.
  • Ability to work collaboratively with healthcare professionals, faculty, staff, and students.
  • Proficiency with electronic health records, learning management systems, and other educational and clinical technologies.

Nice To Haves

  • Completion of a PGY2 Ambulatory Care Residency.
  • Board certification (e.g., BCACP).

Responsibilities

  • Coordinate and deliver didactic, laboratory, and experiential instruction within the Pharm.D. curriculum, with emphasis on ambulatory care, population health, and evidence-based pharmacotherapy.
  • Develop, assess, and continuously improve course materials in alignment with programmatic outcomes and ACPE standards.
  • Facilitate active learning strategies and innovative educational approaches in both face-to-face and online learning environments.
  • Serve as a faculty advisor and mentor to Pharm.D. students.
  • Maintain an active ambulatory care clinical practice that advances patient care and supports the mission of the School and affiliated healthcare partners.
  • Provide direct patient care services such as comprehensive medication management, chronic disease management, preventive care, and transitions of care.
  • Precept and mentor pharmacy students during IPPE and APPE rotations.
  • Participate in interprofessional healthcare teams and collaborative practice initiatives.
  • Engage in scholarly activities that contribute to the advancement of pharmacy practice, pharmacy education, and/or ambulatory care.
  • Disseminate scholarly work through presentations, publications, grant activities, or other recognized scholarly endeavors.
  • Maintain a scholarly agenda consistent with expectations for appointment renewal and promotion.
  • Participate in School, University, and departmental committee assignments as required.
  • Contribute to professional organizations and pharmacy practice initiatives at the local, state, or national level.
  • Engage in community outreach and service activities consistent with the mission and values of D’Youville University.
  • Support accreditation, assessment, recruitment, and other strategic initiatives of the School of Pharmacy.
