We are currently seeking a motivated, organized, and relationship-driven External Client Care Coordinator to support client partnerships, coordinate services, and ensure outstanding care delivery across our service areas.

Responsibilities

Build and maintain strong relationships with clients, families, referral partners, and community organizations
Serve as a trusted liaison between clients and internal care teams
Conduct client consultations and assist with developing personalized service plans
Monitor client satisfaction and proactively address concerns or changing needs
Coordinate service requests, staffing needs, and ongoing care adjustments
Foster partnerships with healthcare professionals, social workers, and community resources
Support client retention through consistent communication and quality follow-up
Assist with growth initiatives, outreach activities, and community networking opportunities
Ensure compliance with company standards and documentation requirements
Collaborate with multiple departments to deliver seamless client experiences
