About The Position

Benchmark Human Services has grown to become one of the most respected leaders in the country in the areas of intellectual and developmental disabilities (IDD) and behavioral health. We work with people of all ages at home, at work, and in the community through residential programs, employment services, crisis response, early intervention, and more. Join Benchmark as an Extended Services Case Coordinator to provide support to individuals in sustaining long-term employment following the closure of their vocational rehabilitation case. Must be willing to travel the greater Fort Wayne area as well as Marion.

Requirements

Bachelor's degree in a related field and/or one (1) year experience preferred with people with disabilities
Three (3) years experience of continued and verifiable employment within two years of submitting an application
Valid Indiana Driver’s license and auto insurance
Reliable transportation

Responsibilities

Maintain accurate client records as per program and regulatory requirements.
Conduct regular meetings with individuals and employers on-site.
Offer support in navigating workplace dynamics and fostering relationships.
Facilitate re-referrals to vocational as necessary.
Provide outcome data to designated personnel.
Monitor and summarize monthly billing activities.
Perform periodic file reviews for accuracy and timeliness.
Coordinate all business between department and state funding bodies.
Oversee client programming and services.
Adhere to attendance requirements set by the program, Benchmark, and individual clients.
