EXTENDED CARE LIAISON
About The Position

We offer a full continuum of care from medical detox to recovery supportive housing with treatment options in the St. Louis area. The intimate settings provided by Sana Lake cultivate an environment that facilitates meaningful recovery and promotes an overall sense of well-being. Sana Lake is expanding rapidly and is seeking an Extended Care Liaison who is passionate about helping individuals and families recover!
Requirements
- High School Diploma or equivalent, preferred
- Members for Life Experience (Members Helping Members Program, additional experience preferred).
- General understanding of substance use disorder and Recovery Programs.
- Proficiency in the Microsoft Office suite of Tools
Responsibilities
- Support Sana Lake Members for Life [M4L] (Alumni) and the Members Helping Members (MHM) program in sustaining recovery through ongoing engagement, mentorship, and events.
- Provide support to the M4L and MHM programs to provide encouragement and support to alumni and current members through follow-up calls, resources, and active participation in recovery-focused programming.
- Track engagement of members.
- Support the mentorship program coordination.
- Support the Wellness for Life Program.
- Support the Outreach program.
- Assist with the M4L events.
- Support the Extended Care Coordinator to maintain an active, supportive alumni network that fosters friendships, provides recovery role models, and reinforces personal recovery goals.
