This leader is responsible for ensuring seamless patient transitions between healthcare services, driving quality outcomes, and maximizing patient experience. The Director champions interdisciplinary collaboration and service excellence, leading initiatives that align care delivery with the health system’s strategic goals.  Key responsibilities include:  Strategic Leadership: Develop and execute care Metro region management strategies that support value-based care, reduce readmissions, and optimize length of stay across multiple locations; Operations Management: Oversee Metro daily operations of care coordination teams, ensuring standard practices and compliance with state/national regulatory guidelines; Patient Experience: Drive initiatives that ensure a seamless, high-quality patient journey through the continuum of care;  Team Development: Mentor and guide staff on complex clinical issues and HR-related regulations; and Collaboration: Foster strong partnerships with clinical leadership, social work, and nursing teams to promote an interdisciplinary approach to care. Requirements

  • Bachelor's Degree is required.
  • Requires 10+ years of relevant experience.
  • Bachelor’s Degree in Nursing (BSN) required; Master’s Degree in Nursing (MSN) preferred or Master’s Degree in Social Work (MSW) required.
  • Active Registered Nurse (RN) license in the state of Indiana (or active Nurse Licensure Compact license) or Licensed Clinical Social Worker (LCSW) required for Social Work candidates.
  • 7-10 years of relevant clinical or case management experience.
  • Including 5+ years of progressive leadership experience (Manager level or above).
  • Strong knowledge of Case Management principles, community resources, and venues for care delivery.
  • Ability to analyze data to drive performance improvement.

Nice To Haves

  • Master's Degree is preferred.
  • Master’s Degree in Nursing (MSN) preferred
