About The Position

The Emergency Room Follow-up Coordinator is primarily responsible for assisting Millennium patients in obtaining appropriate care in the appropriate setting. This position will work with offices, care management staff, and others to educate and listen to patients who may not be seeking care in the most appropriate setting and assisting with the patient being seen in the primary care physician's office as appropriate. S/he must exhibit strong interpersonal and communication skills as well as the ability to utilize both internal and external resources to accomplish the goals and objectives.

Requirements

  • LPN or Medical Assist Certified with one to two years related experience and/or training; or equivalent combination of education and experience.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Ability to write routine reports and correspondence.
  • Ability to speak effectively before groups of customers or employees of organization.
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.
  • Ability to deal with problems involving several concrete variables in standardized situations.
  • Knowledge of various Electronic Health Record software systems; Population Health Software Microsoft Office – Excel, Word, Outlook, etc.

Responsibilities

  • Review information obtained from the Event Notification System, or other information, to assist in the evaluation of whether patients are obtaining care in the most appropriate setting
  • Organize the available information in such a manner or as directed, to share the resources with other Millennium employees to assist the patient as appropriate
  • Support Millennium's employees in the coordination of the care to effective support the patient's care plan
  • Analyze reports and communicate the analysis of the reports with Millennium staff.
