About The Position

The Health Enrollment Specialist supports the mission of Servicios de La Raza by helping individuals, youth, and families gain access to affordable health care coverage. This role exercises independent judgment in assessing clients’ healthcare benefit needs, providing enrollment guidance, and coordinating referrals to ensure equitable access to Medicaid/CHP+, marketplace subsidies, and other low-cost programs. The Health Enrollment Specialist also develops strategies for outreach and education, tracks enrollment outcomes, and maintains accurate documentation. By bridging gaps in coverage and advocating for underserved communities, this position strengthens client health outcomes and contributes to the agency’s broader mission of equity and community well-being.

Requirements

  • High School Diploma or equivalent
  • Two years of experience in a community-serving non-profit setting preferred

Responsibilities

  • Assess the healthcare benefit program needs of individuals, youth, and families, applying professional judgment to determine eligibility and appropriate programs
  • Facilitate enrollment in public health care assistance programs, including Medicaid/CHP+, marketplace subsidies, and other low-cost options
  • Provide in-person, virtual, and phone assistance regarding health insurance options, exercising discretion to tailor guidance to individual client circumstances
  • Plan and facilitate community education regarding health benefit options and the Affordable Care Act (ACA), adapting materials and approach to community needs
  • Facilitate linkage to care through appropriate internal and external referrals, including certified brokers and other community resources
  • Support clients in the referral process for appeals, grievances, or complaints regarding healthcare to appropriate external agencies
  • Develop follow-up plans to ensure complete enrollment, retention, and identification of gaps in healthcare coverage
  • Maintain knowledge of state coverage changes, policies, procedures, and resource availability, applying this knowledge to guide client decisions
  • Accurately track and record all health enrollment applications and outcomes, ensuring compliance with agency standards
  • Screen Coloradoans for SSI/SSDI eligibility
  • Assist Coloradoans with a disabling condition with the completion and submission of SSI/SSD applications
  • Screen Coloradoans with a disabling condition for eligibility of additional Federal, State, and/or Local Benefits, such as SNAP and Medicaid, while screening for SSI/SSDI eligibility.
  • Provide input and recommendations on programmatic improvements based on client and community data
  • All members of the SDLR workforce are expected to support the agency’s community outreach, civic engagement, and recruitment efforts
  • Support the agency’s fundraising objectives, which may include tasks such as active fundraising, grant development, solicitation of donations, agency ambassadorship, informational outreach, and public speaking
  • Commitment to and experience with equity, cultural relevance, diversity, social justice, and disparity reduction practices with persons of color, agencies of color, and underserved communities
  • Ability to establish and maintain effective working relationships with clients, co-workers, agency partners, and the public
