About The Position

Schedule: M-F, 8a-5p, Occasional weekends Salary: $58,000 - $65,000 (based on experience) This position will oversee both sites

Requirements

Bachelor's Degree in humanities/health and human services or equivalent experience with C.N.A. or LPN certification.
Two (2) years of experience in marketing, with more emphasis on sales, and/or admissions role for a health care provider, or in an information and referral resource role within the senior networks.
Comfortable working in a fast pace environment
Excellent customer service

Nice To Haves

General knowledge of Medicare, Medicaid, insurance, VA
Skilled nursing experience

Responsibilities

Under the supervision of the Marketing Manager, obtains initial information relating to admissions, coordinates intake process, and provides information and referrals to community agencies as appropriate.
Promotes the Senior CommUnity Care Program to potential participants and community agencies.
Ability to process and complete admissions
Provide pre-admissions counseling, information and tours
Review EPIC for proper documentation and orders
Facilitate PAS Screening and Level II as needed
Ability to assists Social Services team with various tasks
Ability to participate in Vulnerable Adult investigations and complete necessary documents/reports
