About The Position

We’re hiring an ECM Case Manager in San Jose, California who wants a weekday schedule, meaningful community impact, and the opportunity to help high-risk Medi-Cal members overcome barriers to care. You’ll step into a community-based clinical role focused on outreach, enrollment, assessment, individualized care planning, advocacy, and resource navigation. You’ll work independently in the community while collaborating with multidisciplinary clinical teams and community partners.

Requirements

• Bachelor’s degree in a healthcare or behavioral health-related field, or equivalent training and experience.
• 1+ year of case management or related experience.
• Comfortable working independently and driving throughout the community.
• Valid California driver’s license, personal auto insurance, and acceptable MVR.
• Organized, resourceful, culturally responsive, and comfortable supporting complex needs.

Nice To Haves

• Trauma-informed experience.
• Bilingual ability.

Responsibilities

• Outreach, enrollment, assessment, individualized care planning, advocacy, and resource navigation for high-risk Medi-Cal members.
• Work independently in the community.
• Collaborate with multidisciplinary clinical teams and community partners.
• Conduct outreach across homes, shelters, schools, residential settings, and community locations.
