About The Position
Epic Care is a physician-led, multi-specialty medical group with over 60 physicians and Advanced Practice Practitioners (APPs) serving the East Bay of the San Francisco Bay Area. Their mission is to deliver high-quality, personalized, and value-driven care to patients and their families by offering multiple locations and comprehensive services. The organization is seeking a dedicated and collaborative Advanced Practice Practitioner (Nurse Practitioner or Physician Assistant) to work closely with an Endocrinologist at their San Leandro and Dublin clinic locations. The ideal candidate should have a strong foundation in general medicine, a drive for continuous learning, adaptability, energy, and the ability to thrive in a team-based care model.
Requirements
- Master of Science Degree, Graduate of accredited Nurse Practitioner program. Specialty certification required.
- Baccalaureate in Nursing
- Current CA licensure.
Responsibilities
- Provide follow-up and ongoing management care to patients with endocrine disorders, including diabetic patients
- Medication management for glucose, blood pressure, and cholesterol control
- Ongoing communication with referring clinicians
- Developing expertise in diabetes and endocrine disorder management
- Managing an assigned patient caseload in partnership with physicians and the healthcare team
- Coordinating patient care over time and across healthcare settings to optimize health outcomes
