About The Position

We are currently seeking an Eldercare Manager to manage and coordinate the day-to-day operations of Sheltering Arms, provide nursing services to the residents according to their needs, and provide Public Health nursing services and education to area providers, partner agencies, potential referral sources, and residents in the community. The candidate who fills this role will partake in the development and execution of plans, initiatives, programs, & materials for community-based health programs &/or networking aimed at the prevention of disease and promotion of health; coupled with raising community awareness of UCFS programs and services.

Requirements

  • Bachelor’s Degree in Nursing or a graduate of an accredited school of Licensed Practical Nursing
  • 1-3 year’s experience with the eldercare population
  • Current RN or LPN license in the State of Connecticut
  • Proven experience in eldercare, geriatrics, or long-term care nursing management
  • Strong leadership and team management skills
  • Excellent communication and interpersonal abilities
  • Knowledge of healthcare regulations, compliance standards, and quality assurance processes
  • Ability to assess complex clinical situations and make sound decisions
  • Organizational skills with attention to detail and multitasking capabilities
  • Compassionate, patient-focused approach to eldercare

Nice To Haves

  • Supervisory, management and/or operational experience strongly preferred

Responsibilities

  • Manage and coordinate the day-to-day operations of Sheltering Arms.
  • Provide nursing services to residents according to their needs.
  • Provide Public Health nursing services and education to area providers, partner agencies, potential referral sources, and residents in the community.
  • Partake in the development and execution of plans, initiatives, programs, & materials for community-based health programs &/or networking aimed at the prevention of disease and promotion of health.
  • Raise community awareness of UCFS programs and services.
  • Manage the day-to-day operations of Sheltering Arms and the Supervisor of Resident Attendants.
  • Carry out managerial responsibilities in accordance with the organization’s policies and applicable laws.
  • Lead the daily operations of SA including but not limited to achievement of program goals and objectives in accordance with UCFS, Federal, State and other policies, guidelines and applicable laws.
  • Participate in program planning, budgeting and evaluating programs development and services.
  • Be available for consultation and emergencies occasionally as needed.
  • Support Resident Attendant Supervisor to coordinate scheduling of staff to ensure proper levels of coverage.
  • Conduct hiring, orientation, training, coaching/counseling, discipline, performance management and appraisals.
  • Manage time and attendance.
  • Instruct and coordinate program assignments.
  • Address complaints and resolve problems, and related routine personnel operations.
  • Screen prospective residents and collect data to determine appropriateness of admission to the facility.
  • Coordinate all admissions and discharges.
  • Maintain and update waiting list and all other related activities.
  • Assess and monitor residents’ health changes.
  • Follow-up as appropriate with family members, care givers, physicians, treatment providers and involved agencies.
  • Review residents’ medications, provide oversight, surveillance for compliance with administration and documentation of medications at least monthly or more often as needed.
  • Work in collaboration with residents’ providers and Sheltering Arms selected pharmacy to ensure receipt of provider’s orders and accuracy of fulfillment of medications.
  • Complete and maintain appropriate records and reporting, including resident documentation, statistical data, attendance records, and all required documentation and reporting to ensure licensing requirements are met.
