About The Position

Sanford Health, the largest rural health system in the United States, is dedicated to transforming the health care experience and providing access to world-class health care in America’s heartland. The care coordinator is responsible for providing direct coordination of services to patients and families as directed by the comprehensive assessment, initial service plan and/or treatment plan in support of the treatment team. The Care Coordinator supports the Inpatient Eating Disorder Unit through coordination of admissions, referrals, and non‑counseling case management services. This role is responsible for managing intake inquiries and referral calls, coordinating admissions, obtaining prior authorizations, and supporting the unit social worker as needed. The Care Coordinator uses multiple modes of communication, the Care Coordinator connects with patients, families, and referral sources to support treatment engagement and successful outcomes.

Requirements

  • A minimum of two years of college in a healthcare related field is required.
  • CPR certification required.
  • AED certification required.

Nice To Haves

  • Bachelor’s degree is preferred.
  • A minimum of two years’ experience, preferably working within a behavioral health clinic or medical facility.
