About The Position

Lutheran Services Florida (LSF) is seeking a talented Early Childhood Service Coordinator to provide case management services for families with children aged birth to 6. This role involves coordinating health, wellness, and safety services, managing support plans, and continuously assessing family needs. The position requires engaging families in early childhood events, conducting home visits, developing individualized family support plans, and making timely referrals to community services. The coordinator will also partner with families to strengthen parent-child relationships and support networks, maintain contact with service providers, and provide community education through public presentations. A key aspect of this role is developing and maintaining knowledge of community resources, program eligibility, and emergency procedures within Hillsborough County. The position requires accurate documentation of all case activities and maintenance of case files. Collaboration with volunteers and community resources is essential, as is adherence to Children's Board policies. The role also involves cross-training for daily operating procedures, participation in Center events, and supporting contractual outcomes. Bilingual employees may be needed for translation services, and all employees must attend mandatory trainings. The ideal candidate will demonstrate sensitivity to the cultural and socioeconomic characteristics of the service population, understand the Protective Factors framework, and practice Family Support Principles.

Requirements

  • Bachelor’s Degree in Social Work, Early Childhood Education or related field required.
  • Must have experience in primary responsibility to provide case management for at least 30 open cases at any given time with families who children ages birth to 6.
  • Bilingual English/Spanish required.
  • Must demonstrate sensitivity to our service population’s cultural and socioeconomic characteristics and needs.
  • Understand the value and foundation of the Protective Factors framework.
  • Practice Family Support Principles.
  • Must maintain confidentiality of all client information.
  • Must be trained in the Center data system: Attendance and Needs Tracking System and maintain member confidentiality.

Responsibilities

  • Provide case management services for at least 30 open cases at any given time with families who have children ages birth to 6.
  • Maintain case ownership responsibility for coordinating services and supports identified for each child and family member to include health, wellness and/or safety.
  • Manage the support plan, as required, and continuously monitor and assess the family’s condition and dynamics to make necessary assessment and coordinate supports.
  • Plan early childhood events at each Center to engage families and refer to St. Joseph’s Hospital for developmental screenings.
  • Conduct home visits to meet the needs of the family and ensure successful enrollment in key services.
  • Work with parents/caregivers to develop individualized family support plans.
  • Provide families with timely referrals to services.
  • Partner with families to build strong parent-child relationships and strengthen their support networks with other parents and community members.
  • Maintain regular contact with service providers and document service progress.
  • Provide community education through public presentations.
  • Organize, prioritize and complete all work assignments by the established deadlines.
  • Develop and maintain knowledge of community resources, program eligibility requirements, key contact persons, emergency procedures, and waiting lists of available resources.
  • Work collaboratively with volunteers and community resources.
  • Accurately document all case activities and maintain the case file in accordance with the standardized case file format.
  • Develop and maintain relationships and partnerships with local programs and offer parents/caregivers information and materials regarding community programs.
  • Attend all appointments, staff meetings, trainings, seminars, workshops, etc., as necessary and as required by supervisor.
  • Work flexible hours to include some evenings and Saturdays.
  • Other duties as assigned.
  • Cross trained to perform daily operating procedures.
