About The Position

Lutheran Services Florida (LSF) is seeking a dedicated Early Childhood Service Coordinator to provide case management services for families with children aged birth to 6. This role involves coordinating health, wellness, and safety services, managing support plans, and continuously assessing family needs. The coordinator will also plan early childhood events, conduct home visits, develop individualized family support plans, and make timely referrals to essential services. A key aspect of this position is partnering with families to foster strong parent-child relationships and build supportive community networks. The role requires maintaining regular contact with service providers, documenting progress, and providing community education through public presentations. The Early Childhood Service Coordinator must be organized, able to meet deadlines, and possess a strong understanding of community resources, including Hillsborough County child-serving systems and Children's Board policies. Accurate documentation and case file maintenance are essential. The position also involves developing and maintaining relationships with local programs and offering parents information on community resources. Attendance at appointments, staff meetings, and trainings is required, along with the flexibility to work some evenings and Saturdays. The role demands sensitivity to the cultural and socioeconomic needs of the service population, an understanding of the Protective Factors framework, and adherence to Family Support Principles. Cross-training for daily operating procedures and participation in Center events to support contractual outcomes are also expected. Familiarity with the Center data system (Attendance and Needs Tracking System) and maintaining member confidentiality are crucial. Bilingual employees may be utilized for translation services. The position requires a high level of energy, adaptability to irregular hours, and the ability to travel within Hillsborough County.

Requirements

  • Bachelor’s Degree in Social Work, Early Childhood Education or related field required.
  • Must have experience in primary responsibility to provide case management for at least 30 open cases at any given time with families who children ages birth to 6.
  • Bilingual English/Spanish required.
  • Must demonstrate sensitivity to our service population’s cultural and socioeconomic characteristics and needs.
  • Understand the value and foundation of the Protective Factors framework.
  • Practice Family Support Principles.
  • Must maintain confidentiality of all client information.

Responsibilities

  • Provide case management services for at least 30 open cases at any given time with families who have children ages birth to 6.
  • Maintain case ownership responsibility for coordinating services and supports identified for each child and family member to include health, wellness and/or safety.
  • Manage the support plan, as required, and continuously monitor and assess the family’s condition and dynamics to make necessary assessment and coordinate supports.
  • Plan early childhood events at each Center to engage families and refer to St. Joseph’s Hospital for developmental screenings.
  • Conduct home visits to meet the needs of the family and ensure successful enrollment in key services.
  • Work with parents/caregivers to develop individualized family support plans.
  • Provide families with timely referrals to services.
  • Partner with families to build strong parent-child relationships and strengthen their support networks with other parents and community members.
  • Maintain regular contact with service providers and document service progress.
  • Provide community education through public presentations.
  • Organize, prioritize and complete all work assignments by the established deadlines.
  • Develop and maintain knowledge of community resources, program eligibility requirements, key contact persons, emergency procedures, and waiting lists of available resources.
  • Work collaboratively with volunteers and community resources.
  • Accurately document all case activities and maintain the case file in accordance with the standardized case file format.
  • Develop and maintain relationships and partnerships with local programs and offer parents/caregivers information and materials regarding community programs.
  • Attend all appointments, staff meetings, trainings, seminars, workshops, etc., as necessary and as required by supervisor.
  • Work flexible hours to include some evenings and Saturdays.
  • Other duties as assigned.
  • Cross trained to perform daily operating procedures.
  • Participate in Center events and work to support contractual outcomes.
  • Must be trained in the Center data system: Attendance and Needs Tracking System and maintain member confidentiality.
  • Bilingual employees may be needed to review and/or translate (verbal or written communications).
