About The Position

Lutheran Services Florida (LSF) is seeking a talented Early Childhood Service Coordinator to provide case management services for families with children ages birth to 6. This role involves coordinating services and supports for health, wellness, and safety, managing support plans, and continuously monitoring family conditions. The position requires engaging families in early childhood events, conducting home visits, developing individualized family support plans, and making timely referrals to services. The coordinator will partner with families to build strong parent-child relationships and support networks, maintain contact with service providers, and provide community education. This role demands organization, prioritization, and timely completion of work assignments, along with developing and maintaining knowledge of community resources. Collaboration with volunteers and community resources is essential, as is knowledge of Hillsborough County child-serving systems and community resources, and Children's Board policies. Accurate documentation of case activities and maintenance of case files are critical. The role also involves developing and maintaining relationships with local programs, attending appointments and meetings, and working flexible hours, including some evenings and Saturdays. The ideal candidate will demonstrate sensitivity to the cultural and socioeconomic characteristics and needs of the service population, understand the Protective Factors framework, and practice Family Support Principles. Cross-training for daily operating procedures, participation in Center events, and supporting contractual outcomes are also expected. Training in the Center data system (Attendance and Needs Tracking System) and maintaining member confidentiality are required. Bilingual employees may be needed for translation. Mandatory trainings must be attended, and other duties as assigned.

Requirements

• Bachelor’s Degree in Social Work, Early Childhood Education or related field required.
• Must have experience in primary responsibility to provide case management for at least 30 open cases at any given time with families who children ages birth to 6.
• Bilingual English/Spanish required.
• Must demonstrate sensitivity to our service population’s cultural and socioeconomic characteristics and needs.
• Understand the value and foundation of the Protective Factors framework.
• Practice Family Support Principles.
• Must be trained in the Center data system: Attendance and Needs Tracking System.
• Must maintain confidentiality of all client information.
• Accurate complete and timely submission of required documents and reports.
• Adherence to agency policies and procedures.

Responsibilities

• Provide case management services for at least 30 open cases at any given time with families who have children ages birth to 6.
• Maintain case ownership responsibility for coordinating services and supports identified for each child and family member to include health, wellness and/or safety.
• Manage the support plan, as required, and continuously monitor and assess the family’s condition and dynamics to make necessary assessment and coordinate supports.
• Plan early childhood events at each Center to engage families and refer to St. Joseph’s Hospital for developmental screenings.
• Conduct home visits to meet the needs of the family and ensure successful enrollment in key services.
• Work with parents/caregivers to develop individualized family support plans.
• Provide families with timely referrals to services.
• Partner with families to build strong parent-child relationships and strengthen their support networks with other parents and community members.
• Maintain regular contact with service providers and document service progress.
• Provide community education through public presentations.
• Organize, prioritize and complete all work assignments by the established deadlines.
• Develop and maintain knowledge of community resources, program eligibility requirements, key contact persons, emergency procedures, and waiting lists of available resources.
• Work collaboratively with volunteers and community resources.
• Accurately document all case activities and maintain the case file in accordance with the standardized case file format.
• Develop and maintain relationships and partnerships with local programs and offer parents/caregivers information and materials regarding community programs.
• Attend all appointments, staff meetings, trainings, seminars, workshops, etc., as necessary and as required by supervisor.
• Work flexible hours to include some evenings and Saturdays.
• Cross trained to perform daily operating procedures.

• Maintain member confidentiality.
