About The Position

Lutheran Services Florida (LSF) is seeking a talented Early Childhood Service Coordinator to provide case management services for families with children ages birth to 6. This role involves coordinating services and supports for health, wellness, and safety, managing support plans, and continuously assessing family conditions. The coordinator will engage families, conduct home visits, develop individualized family support plans, provide referrals, build parent-child relationships, and maintain contact with service providers. Community education through public presentations is also a key function. The role requires organizing and prioritizing work to meet deadlines, maintaining knowledge of community resources, and collaborating with volunteers and community resources. Familiarity with Hillsborough County child serving systems and community resources, as well as Children's Board policies, is necessary. Accurate documentation of case activities and maintenance of case files are essential. The coordinator will also develop and maintain relationships with local programs and offer parents information on community programs. Attendance at appointments, staff meetings, trainings, and seminars is required. The position involves working flexible hours, including some evenings and Saturdays, and performing other duties as assigned. Bilingual employees may be needed for translation. The role requires sensitivity to the cultural and socioeconomic characteristics and needs of the service population, understanding the Protective Factors framework, practicing Family Support Principles, and cross-training for daily operating procedures. Participation in Center events and support of contractual outcomes is expected, along with training in the Center data system and maintaining member confidentiality.

Requirements

  • Bachelor’s Degree in Social Work, Early Childhood Education or related field required.
  • Must have experience in primary responsibility to provide case management for at least 30 open cases at any given time with families who children ages birth to 6.
  • Bilingual English/Spanish required.
  • Must demonstrate sensitivity to our service population’s cultural and socioeconomic characteristics and needs.
  • Understand the value and foundation of the Protective Factors framework.
  • Practice Family Support Principles.
  • Cross trained to perform daily operating procedures.
  • Participate in Center events and work to support contractual outcomes.
  • Reports to Service Coordinator Supervisor.
  • Team player with co-workers and administrators.
  • Accurate complete and timely submission of required documents and reports.
  • Liaison to community providers.
  • Adherence to agency policies and procedures.
  • Must maintain confidentiality of all client information.

Responsibilities

  • Provide case management services for at least 30 open cases at any given time with families who have children ages birth to 6.
  • Maintain case ownership responsibility for coordinating services and supports identified for each child and family member to include health, wellness and/or safety.
  • Manage the support plan, as required, and continuously monitor and assess the family’s condition and dynamics to make necessary assessment and coordinate supports.
  • Plan early childhood events at each Center to engage families and refer to St. Joseph’s Hospital for developmental screenings.
  • Conduct home visits to meet the needs of the family and ensure successful enrollment in key services.
  • Work with parents/caregivers to develop individualized family support plans.
  • Provide families with timely referrals to services.
  • Partner with families to build strong parent-child relationships and strengthen their support networks with other parents and community members.
  • Maintain regular contact with service providers and document service progress.
  • Provide community education through public presentations.
  • Organize, prioritize and complete all work assignments by the established deadlines.
  • Develop and maintain knowledge of community resources, program eligibility requirements, key contact persons, emergency procedures, and waiting lists of available resources.
  • Work collaboratively with volunteers and community resources.
  • Accurately document all case activities and maintain the case file in accordance with the standardized case file format.
  • Develop and maintain relationships and partnerships with local programs and offer parents/caregivers information and materials regarding community programs.
  • Attend all appointments, staff meetings, trainings, seminars, workshops, etc., as necessary and as required by supervisor.
  • Work flexible hours to include some evenings and Saturdays.
  • Other duties as assigned.
  • Be trained in the Center data system: Attendance and Needs Tracking System and maintain member confidentiality.
  • Attend mandatory trainings and perform other duties as assigned.
