About The Position

The DSS Community Engagement Navigator serves as a vital link between the health plan, local Departments of Social Services (DSS), and community stakeholders. The role supports North Carolina’s Children and Families Specialty Plan (CFSP), a statewide Medicaid plan intended to provide coordinated health care and consistent access to benefits and services for Medicaid-enrolled infants, children, youth, young adults, and families served by the child welfare system. Covered support areas include physical and behavioral health, pharmacy, intellectual and developmental disability services, long-term services and supports, unmet health-related resource needs, and integrated care management. The Navigator engages members with significant social determinants of health (SDOH) barriers, helps them access services and resources, and works with members, interdisciplinary teams, Care Managers, and external stakeholders to identify goals and develop aligned care plans. The role is virtual full-time, with required in-person training sessions and a requirement to be within reasonable commuting distance of the posting location unless a legally required accommodation applies. The position has a structured Monday-through-Friday schedule, 8:00 a.m. to 5:00 p.m. Eastern time, with occasional overtime or flexibility based on business needs and the ability to work from an office as needed. The position requires managing multiple communication channels and electronic queues while taking notes and speaking with customers, maintaining focus during extended periods of sitting, and communicating effectively in virtual and in-person settings. Other duties may be assigned.
