About The Position

The Community Navigator works with treatment teams at the Youth Behavioral Hospital to provide short-term care coordination and connect individuals and their guardians with resources and support as they transition from inpatient to outpatient care. As the position grows, the Community Navigator will serve as a resource for individuals and families navigating the health care system.
