About The Position

HealthChoice Waiver Services is seeking compassionate, dependable IHS staff to support adults with disabilities in their homes and communities. The role provides individualized, person-centered assistance to help clients build independence, pursue personal growth, participate in the community, and maintain a safe, comfortable home environment. The position involves following individualized service plans, documenting services accurately, and communicating respectfully with clients, families, and staff. Scheduling must be flexible to accommodate changing schedules and client needs.

Requirements

• Valid driver’s license and reliable transportation.
• Willingness to complete required training before providing services.
• Ability to be flexible with scheduling.
• Strong interpersonal skills and ability to work independently.
• Compassion, patience, and respect for individuals with disabilities.

Responsibilities

• Assist clients with daily living skills, including meal preparation, light cleaning, and organization.
• Support clients in developing and practicing independent living skills.
• Provide transportation to appointments, errands, or community activities as needed.
• Follow individualized service plans and maintain accurate documentation.
• Communicate professionally and respectfully with clients, families, and staff.
• Adapt to changing schedules and client needs.
