About The Position

Carolina SeniorCare is a P.A.C.E. (Program of All-inclusive Care for the Elderly). A career at Carolina SeniorCare is a calling to help others in their community and help elderly participants (55 or older) stay and live in their respective communities. P.A.C.E. offers a wide variety of services to fulfill this mission such as medical services to medications, from nutritious meals to transportation – all personalized to meet the participant’s specific needs!

Requirements

  • High school diploma or equivalent required.
  • Cardiopulmonary Resuscitation (CPR)/First Aid certification required.
  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
  • Must have a valid driver’s license, proof of insurance and have means of transportation.
  • Must be able to meet Carolina SeniorCare's auto insurance carrier's standards for coverage which requires a minimum age of 23 years old.
  • A minimum of one year of experience in working with the frail or elderly population required.

Responsibilities

  • Responsible for EveryAge PACE passengers during transport, providing care in a manner which ensures participant satisfaction.
  • Follows assigned schedule, makes adjustments to meet unanticipated changes, while considering the needs of the participant and requirements of the PACE Center. Communicates these changes to dispatch.
  • Maintains and completes daily required records i.e., Pre- and Post-Trip Inspections, gasoline purchases.
  • Transports participants to and from medical/specialist appointments, and activity outings, as requested by the Interdisciplinary Team via the plan of care.
  • Provides physical assistance to each participant when needed as he/she boards and debarks from the program’s vehicles.
  • Communicates with Transportation Coordinator daily to be updated on participant conditions/ changes, plan of care changes, or Interdisciplinary Team requests.
  • Participates in continuing education classes and any required staff and training meetings. Maintains any required certifications or licenses
  • Performs other duties as required or requested.
