This position will collaborate with other professionals to evaluate patients' medical or physical conditions and to assess client needs as part of the interdisciplinary team. They work collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plans of care for both individual patients and the medical community. This role involves providing psychosocial assessments of patients and families with social, emotional, interpersonal, and/or environmental issues, and formulating biopsychosocial assessment, disposition, and treatment plans. The advocate will monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan.
