The Case Manager II provides comprehensive assessment, case management, treatment planning, restorative justice facilitation, and participant support throughout the diversion process. The position works with participants, families, internal team members, and community partners to support successful completion of individualized diversion plans. Key functions include conducting assessments and screenings, coordinating services, maintain participants’ records, monitoring progress, participating in multidisciplinary meetings, identifying and fostering professional relationships with other community-based providers, connecting diversion participants with community resources and providers as needed, providing educational advocacy and supporting restorative justice practices. Duties may include clinical assessment and therapeutic services when the employee meets required qualifications.
