Diversion Case Manager (Behavioral Health Specialist)
About The Position
The Department of Human Services is seeking a Diversion Case Manager to join their team. This role is part of the Jail Diversion/Forensic Case Management program, providing mental health assessments and case management services for adults with mental health and/or co-occurring mental health and substance use conditions within the Jail Diversion Program. The position will also provide as-needed services to other individuals served on the Forensic Jail Diversion Team. The Case Manager will work as an integral part of the team to assess participant needs and provide case management services to help individuals achieve goals and enhance skills for successful navigation of the criminal justice system. Responsibilities
- Provide mental health assessments and case management services for adults with mental health and/or co-occurring mental health and substance use conditions within the Jail Diversion Program.
- Provide as-needed services to other individuals served on the Forensic Jail Diversion Team.
- Assess the needs of participants.
- Provide case management services to assist individuals with achieving goals.
- Enhance skills to successfully navigate the criminal justice system.
Benefits
- $2,500.00 gross signing bonus at hire
- $2,500.00 gross bonus at 12 months or upon completion of the probationary period.
