About The Position

Mental Health Kokua (MHK) is hiring a Case Manager in Kona, Hawaiʻi to support adults experiencing mental health challenges through community-based services. This full-time position focuses on helping individuals build stability, strengthen independence, and connect with behavioral health, housing, medical, and community resources that support their recovery goals. As a Behavioral Health Case Manager, you'll build meaningful relationships while helping individuals navigate their recovery journey with compassion and respect. Working independently in the community and alongside a supportive behavioral health team, you'll make a meaningful difference every day by empowering people to achieve greater independence and improve their quality of life.

Requirements

  • Bachelor's degree in psychology, sociology, social work, human services, or a related behavioral health field.
  • One and a half (1.5) years or more of case management, human services, behavioral health, or mental health experience; experience working with individuals experiencing serious mental illness and/or co-occurring challenges is a plus.
  • Strong communication, organization, documentation, advocacy, and relationship-building skills.
  • Current TB clearance, CPR, and First Aid certification (or willingness to obtain).
  • Comfortable supporting adults with mental health challenges in community-based settings while promoting recovery, stability, and independence.
  • Ability to pass a criminal background check and drug/alcohol screening.
  • Valid Hawaiʻi driver's license, current vehicle insurance, and safety inspection.
  • Ability to travel throughout Hawaiʻi Island for client visits, appointments, meetings, and community-based services.
  • Ability to lift up to 25 pounds and respond appropriately to emergencies as needed.
  • Ability to work a flexible schedule based on program and client needs.

  • Experience working with individuals experiencing serious mental illness and/or co-occurring challenges is a plus.

Responsibilities

  • Assess client needs, develop individualized service plans, and coordinate behavioral health, medical, housing, and community resources.
  • Support individuals in accessing benefits, services, and community-based supports that promote stability and independence.
  • Help individuals maintain housing, strengthen independent living skills, and connect with community resources that support long-term success.
  • Promote recovery using person-centered, strength-based, and trauma-informed practices.
  • Partner with healthcare providers, families, landlords, and community organizations to support client goals.
  • Respond appropriately to client crises and coordinate support services in accordance with agency policies.
  • Complete timely assessments, service plans, progress notes, and required documentation in accordance with agency and funding requirements.
  • Participate in supervision, case reviews, and interdisciplinary team meetings to ensure quality, recovery-focused services.
