About The Position

The Department of Human Services is seeking a Diversion Case Manager to join our team! The Diversion Case Manager is a part of the Jail Diversion/Forensic Case Management program that will provide mental health assessments and case management services for adults with mental health and/or co-occuring mental health and substance use conditions within the Jail Diversion Program, with additional as-needed services being given to other individuals served on the Forensic Jail Diversion Team. The Manager will work as an integral part of the team to assess the needs of participants and provide case management services to assist individuals with achieving goals and enhance skills to successfully navigate the criminal justice system. Requirements

  • Bachelor's degree in Social Work, Sociology, Psychology, Special Education, Mental Health, or Counseling
  • Two years of full-time work experience providing behavioral health services program delivery to include providing case management services and assessments for adults with serious mental illness and co-occurring disorders and linking these clients to available services
  • Virginia Licensure as a Qualified Mental Health Professional (QMHP-A) is required within 60 days of hire
  • Possess, or obtain by time of appointment, a valid driver’s license, and provide or authorize Arlington County to obtain a copy of official driving record
  • Complete a pre-hire federal background check and State of Virginia Central Registry check

  • Master's degree may substitute for the experience requirement
  • Experience in delivering client services in a jail or community-based mental health setting
  • Use of the Sequential Intercept Model and other Best Practices related to forensic mental health
  • Bilingual language skills (Spanish)
  • Conducting assessments of seriously mentally ill to determine needs for outpatient clinical services, housing needs, financial entitlements, and other services
  • Collecting and converting data into statistics to measure program outcomes

Responsibilities

  • Conducting clinical assessments of mental health status and using evidence-based tools to determine housing needs, mental health and/or substance use services
  • Utilizing the outcome data of the RNR (Risk-Needs-Responsivity) Simulator Tool to identify appropriate level/dosage of treatment and implementing the identified plan
  • Attending court dates as required
  • Participating and consulting with criminal justice and community partners for service delivery and care coordination
  • Attending team meetings and providing information to the team and stakeholders regarding participant’s progress and areas of non-compliance
  • Identifying financial entitlements and other services available to meet participant’s' needs
  • Consulting with federal and local agencies regarding clients' treatment needs and facilitating linkage of clients with the full scope of available services and support options
  • Developing and facilitating psychosocial groups
  • Coordinating services provided by others and maintaining case records and related paperwork including progress notes and treatment plans in an electronic database
  • Assisting within the transportation of clients to various appointments
