About The Position
The Discharge Planner is a key member of the Lincoln Trail Behavioral Health System social services team and coordinates discharge planning services within the Social Services Department. Lincoln Trail Behavioral Health System is a 140-bed hospital providing inpatient services for adolescents, adults, military members, veterans, and first responders. Its care is provided by psychiatrists, licensed therapists, certified alcohol and drug counselors, activity therapists, teachers, and registered nurses.
Requirements
- Bachelor’s degree in a Human Services field or equivalent.
- Experience in a healthcare setting.
Responsibilities
- Conduct daily audits to ensure clinical plans are effective.
- Verify communication with appropriate supports and partners.
- Direct discharge-planning and case-coordination assignments.
- Provide reports and develop discharge process improvements.
