About The Position
Prime Healthcare is seeking a Discharge Planner to coordinate activities of the Case Management Department under the direction of an assigned Case Manager or Social Worker. The role supports the development and implementation of discharge plans, maintains and coordinates Medicaid state submission forms for on-site review, and assists with case management referrals and discharge planning. This includes interviewing patients and families, following up on required assignments, and organizing the development of a safe, patient-focused discharge plan.
Requirements
- Knowledge of discharge planning, utilization management, and case management terminology and functions in managed care and non-managed care environments.
- Basic to intermediate computer skills and knowledge.
- High school diploma required.
Nice To Haves
- Current BCLS certificate required upon hire; maintaining a current certificate is preferred.
- Hospital setting experience preferred.
Responsibilities
- Coordinate Case Management Department activities under the direction of an assigned Case Manager or Social Worker in developing and implementing discharge plans.
- Maintain and coordinate Medicaid state submission forms for presentation to an on-site reviewer, and file them according to facility procedures.
- Assist with case management referrals and discharge planning as assigned.
- Interview patients and families, follow up on required assignments, and organize development of safe, patient-focused discharge plans.
