About The Position

Centinela Hospital Medical Center is seeking a Discharge Planner to coordinate activities of the Case Management Department under the direction of an assigned Case Manager or Social Worker. The role assists with developing and implementing discharge plans, case management referrals and authorizations, and Medicaid state submission forms. It also supports discharge planning by interviewing patients and families and gathering information to develop safe, patient-focused discharge plans. Centinela Hospital Medical Center is a 362-bed acute-care hospital serving Inglewood and the wider Los Angeles area. It is part of Prime Healthcare and provides emergency, stroke, orthopedic, cardiac, critical care, and rehabilitation services.

Requirements

Knowledge of Discharge Planning, Utilization Management, and Case Management terminology and functions in managed-care and non-managed-care environments.
Basic to intermediate computer skills and knowledge.
High school diploma.

Nice To Haves

Current BCLS certificate upon hire; maintaining current certification is preferred.
Associate degree or LVN license.
Experience in a hospital setting.

Responsibilities

Coordinate various activities of the Case Management Department under the direction of the assigned Case Manager or Social Worker.
Assist with the development and implementation of discharge plans.
Assist with case management referrals and authorizations.
Maintain and coordinate Medicaid state submission forms for presentation to the on-site reviewer, and file them according to facility procedure.
Assist with discharge planning as assigned by interviewing patients and families and gathering data to develop a safe, patient-focused discharge plan.
