About The Position

The Discharge Planner is responsible for coordinating the discharge process for clients in the Adult Recovery Unit, which serves individuals aged 18-64 who are experiencing mental health and substance use challenges. Located in Waynesville, NC, this 16-bed program focuses on stabilizing clients and preparing them for continued recovery after discharge. The Discharge Planner ensures that each client has a comprehensive and individualized plan that addresses their ongoing care needs in the community. Assessment & Planning: Collaborate with the ARU treatment team to assess each client’s needs and complete individualized discharge assessments that include, but are not limited to mental health, substance use recovery, medical, housing, and social support services. Coordination of Care: Work with clients, families, and community agencies to arrange post-discharge care, including scheduling follow-up appointments, coordinating with rehabilitation facilities and outpatient providers, and arranging transportation. Client Education & Support: Provide clients and their families with information about available resources and services in the community, such as support groups, outpatient treatment, and housing options. Documentation: Maintain accurate and up-to-date discharge planning documentation in the electronic health record system (e.g., Credible) according to unit policies and procedures. Collaboration: Work closely with therapists, nurses, CMHAs, and prescribers to ensure discharge plans address all areas of the client’s recovery, including psychiatric and medical needs. Requirements

  • A high school diploma or equivalent; Must be 18 years or older.
  • One year experience working with populations served preferred
  • Experience and some knowledge and understanding of the needs, problems, and attitudes of persons who experience psychiatric symptoms or challenges with substance abuse.
  • A general knowledge of the Recovery concept, and the ability to assist RNs and appropriate persons to convey this knowledge to clients.
  • Must be authorized to work in the United States.

Responsibilities

  • Assess client needs and develop individualized discharge plans.
  • Coordinate follow-up appointments, referrals, and community services.
  • Educate clients and families on available recovery resources and support options.
  • Complete timely and accurate documentation while collaborating with the treatment team.
