About The Position

The Discharge Planner is responsible for coordinating the discharge process for clients in the Adult Recovery Unit, which serves individuals aged 18-64 who are experiencing mental health and substance use challenges. Located in Waynesville, NC, this 16-bed program focuses on stabilizing clients and preparing them for continued recovery after discharge. The Discharge Planner ensures that each client has a comprehensive and individualized plan that addresses their ongoing care needs in the community. This description outlines the general scope and level of work for this role. It is not intended to include every duty or responsibility that may be required of employees in this position.

Requirements

  • A high school diploma or equivalent; Must be 18 years or older.
  • One year experience working with populations served preferred
  • Experience and some knowledge and understanding of the needs, problems, and attitudes of persons who experience psychiatric symptoms or challenges with substance abuse.
  • A general knowledge of the Recovery concept, and the ability to assist RNs and appropriate persons to convey this knowledge to clients.
  • Must be authorized to work in the United States.

Responsibilities

  • Assess client needs and develop individualized discharge plans.
  • Coordinate follow-up appointments, referrals, and community services.
  • Educate clients and families on available recovery resources and support options.
  • Complete timely and accurate documentation while collaborating with the treatment team.
