Join UPMC St. Margaret Hospital's Clinical Care Coordination Team! As a Discharge Plan Manager with St. Margaret Hospital, you will work either 8 or 10-hour shifts Monday through Friday with occasional weekend requirements based on staffing needs with working hours of 7:00am-3:30pm or 7:00am-5:30pm. Discharge Plan Managers partner closely with patients, families, physicians, nurses, therapists, and other healthcare professionals to develop individualized care plans that support safe and successful transitions from the hospital to the next level of care. From admission through discharge and beyond, you will advocate for patients, assess their unique clinical and psychosocial needs, coordinate post-hospital services, and help remove barriers to recovery. Purpose: The Discharge Plan Manager functions as the coordinator and is accountable for all post-discharge needs and acts as financial steward for the hospital by assessing for relevant factors, engaging with the care team, and placing a focus on an optimal discharge plan with timely utilization of hospital resources. This optimal discharge plan reviews discipline recommendations and coordinates necessary care for positive patient outcomes outside of the inpatient setting.
