About The Position

The Discharge Facilitator plays a critical role in supporting the Social Services department by ensuring a smooth, safe, and efficient patient departure process. Working under the guidance of the Director of Social Services, this role focuses strictly on the tactical execution of the discharge plan created by the care team. The Discharge Facilitator does not create or modify the discharge plan, but is responsible for completing all operational, administrative, and logistical tasks required to transition the patient safely out of the facility.

Requirements

  • High School Diploma or GED required
  • 1–2 years of experience in a hospital, clinical, or social services setting performing administrative or patient-support duties.
  • Experience navigating Electronic Medical Record (EMR) systems (e.g., Epic, Cerner) and general office equipment (faxing, scanning).
  • Exceptional accuracy in managing paperwork, audits, and patient personal effects.
  • Compassionate and patient-focused communication style, with a high degree of professionalism when dealing with external agencies.

Nice To Haves

  • Associate degree in Healthcare Administration, Human Services, or related field preferred.

Responsibilities

  • Arrange and schedule appropriate patient transportation (rideshare, non-emergency medical transportation, or family coordination) as needed to facilitate timely departure.
  • Retrieve patient items from the belongings closet, carefully audit and verify all items for accuracy against the intake inventory, and return them safely to the patient.
  • Complete the official patient sign-out process within the Electronic Medical Record (EMR) system.
  • Close out and finalize all necessary social services forms, tasks, and documentation within the patient’s chart.
  • Compile and audit the physical discharge folder to ensure it entails all accurate, required paperwork prior to departure.
  • Scan and upload all completed, signed discharge paperwork into the patient’s chart promptly following departure.
  • Fax all necessary follow-up paperwork, social service records, and referrals to external agencies.
  • Administer and complete the patient satisfaction survey prior to departure, providing valuable insight to the Social Services team.
