About The Position

Highland Springs, part of Lifepoint Health, is hiring a Discharge Coordinator for its behavioral health hospital. The role provides aftercare coordination for patients who need ongoing medical or psychosocial care after discharge. The coordinator maintains community-resource information, participates in treatment planning, arranges aftercare appointments identified by the provider and therapist, and shares relevant resources with patients and families. The position also involves proactive communication with clients, families, aftercare providers, and referral sources.

Requirements

• Bachelor’s degree in Social Work, Counseling, or Psychology, or at least three years of clinical experience in an inpatient behavioral health setting.
• Prior experience with psychiatric and chemical dependency patients.
• CPR certification and Crisis Prevention Training (CPI) within 30 days.

• Unencumbered clinical license, if eligible or needed under state regulations.

Responsibilities

• Provide aftercare coordination for patients who need ongoing medical or psychosocial care following discharge.
• Create and maintain current community resources for aftercare coordination.
• Participate actively in treatment planning.
• Arrange aftercare appointments identified by the patients’ provider and therapist.
• Provide patients and families with resource information for future care needs.
• Communicate proactively with clients, families, aftercare providers, and referral sources involved in patients’ treatment.
