Responsible for carrying out and managing / supervising other qualified and trained pharmacists in Transitions of Care activities and Clinical and operational Pharmacy services at the site. Promote clinically rational drug therapy and sound pharmaceutical care through the development of new (or expansion of existing) pharmacy practice programs, drug therapy policies, and other programs. Serve as a transitions of care resource for the health-system and its affiliated pharmacy services. Responsible to serve as a preceptor for the PGY-1 residency program. Responsible to serve as a preceptor for 9 months each year for APPE students from Long Island University. Conduct research and other scholarly activities throughout the year with the goal of publishing. Responsible for ensuring that licensed staff members maintain an active New York State registration. Responsible for insuring that a copy of the individual’s license and current registration, as well as a printout of the primary source online verification regarding active registration, is on file in the employee’s folder upon the beginning of the employment and on an ongoing basis prior to expiration. The DOP will ensure that the license and registration are posted in accordance with the regulations of the NYS Board of Pharmacy. It is the responsibility of the employee to register and to advise the Director when registration is complete.
