About The Position

Requirements

• Must possess, as a minimum, a high school diploma or its equivalent.
• Must be able to pass a background check.

Nice To Haves

• Completion Certificate for approved Social Services training program (preferred).
• One (1) or more years or work in a skilled nursing facility, hospice or long-term acute care (preferred).

Responsibilities

• Identify each resident’s social, emotional and psychological needs.
• Develop and carry out a plan to develop the residents’ full potential during their stay.
• Planning for the resident’s discharge or transition to another care environment.
• Develop social assessment and care plan, which identifies medically related social and emotional problems and needs with realistic goals and specific actions to be taken
• Help coordinate pre-admission and admissions processes
• Ability to listen and act compassionately with residents/family members; must be able to work on own and maintain job related jobs
• Document residents’ behavioral and psycho-social status
• Provide individual and group help for family and residents at all times of adjustment, crisis.
• Orients resident and family to Nursing center, helping both adjust to new environment , ensuring they feel welcome.
