About The Position

CareOne at Randolph is seeking a Director of Social Services to join their team. This role involves managing social service functions, providing direct services, advocacy, counseling, and program development within a healthcare setting. The Director will ensure residents and their families receive comprehensive support, cope with social and psychological challenges, and have their rights maintained. This position requires adherence to company policies, state and federal guidelines, and ethical codes.

Requirements

  • Master's degree in social work
  • Current and Active LSW (Licensed Social Worker) licensure as required by the State
  • Practice Setting experience (Sub-Acute Care, Short Term Rehab, Assisted Living, Skilled Nursing, LTACH, Long Term Care)
  • Ability to practice according to CareOne Social Service Policies and Procedures Manual, State and Federal guidelines, and the National Association of Social Workers & Code of Ethics

Responsibilities

  • Supervising social service staff
  • Providing direct services, advocacy, counseling, and service coordination
  • Developing and implementing social service policies and programs
  • Conducting initial evaluations/assessments of residents' psychosocial needs
  • Developing the social service component of the resident's plan of care
  • Participating in and coordinating interdisciplinary care planning conferences
  • Acting as a liaison between residents, families, outside agencies, and the Center Administrator
  • Facilitating discharge planning
  • Ensuring resident safety and adherence to health, safety, fire, and sanitation codes
  • Providing therapeutic intervention to help residents cope with social and psychological aspects of their illnesses, impairments, or disabilities
  • Maintaining written documentation in the resident's medical record
  • Conducting family meetings as needed
  • Performing administrative requirements such as completing necessary forms and reports
