About The Position

We are seeking a compassionate, experienced, and organized Director of Social Services to lead the social services department in our skilled nursing facility. The Director of Social Services is responsible for overseeing the psychosocial well-being of residents, ensuring compliance with federal, state, and local regulations, and providing leadership to support residents and their families throughout their stay. Requirements

  • Bachelor's degree in Social Work (BSW) required
  • Current Ohio Licensed Social Worker (LSW) required
  • Minimum of 2–3 years of social work experience in a skilled nursing facility, long-term care, rehabilitation, or healthcare setting.
  • Thorough knowledge of CMS regulations, discharge planning, resident rights, and long-term care practices.
  • Strong communication, conflict resolution, and organizational skills.

Nice To Haves

  • Master's degree in Social Work (MSW) preferred.
  • Previous supervisory or leadership experience preferred.
  • Experience with electronic health records (EHR) preferred.

Responsibilities

  • Direct and manage all aspects of the Social Services Department.
  • Complete comprehensive psychosocial assessments for new admissions and ongoing resident evaluations.
  • Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team.
  • Provide counseling, emotional support, crisis intervention, and advocacy for residents and families.
  • Coordinate discharge planning and facilitate safe transitions to the next level of care.
  • Assist residents and families with advance directives, financial resources, community services, and long-term care planning.
  • Facilitate resident and family meetings, support groups, and educational programs.
  • Investigate and address resident concerns, grievances, and allegations of abuse or neglect in accordance with facility policies.
  • Maintain accurate, timely, and compliant documentation in the electronic medical record.
  • Ensure compliance with CMS, state, and federal regulations governing long-term care.
  • Collaborate with nursing, rehabilitation, dietary, activities, and medical staff to promote resident-centered care.
  • Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
  • Supervise, mentor, and evaluate social services staff, as applicable.
