About The Position

Legacy Healthcare Services is seeking a Physical Therapist or Occupational Therapist to serve as Director of Rehabilitation at Greeley Village. The role combines direct resident care with leadership of the rehabilitation program. The Director provides individualized therapy while guiding day-to-day operations, developing the caseload, supporting the team, and strengthening relationships in the community. The position includes a 70% team efficiency expectation and participation in a 13-week Pathway to Success leadership development program to support the transition into leadership. The stated pay range is $43.00–$50.00 per hour, with a $10,000 sign-on bonus.

Requirements

• Degree in Physical Therapy or Occupational Therapy.
• Current Physical Therapy or Occupational Therapy license in Colorado, or ability to obtain one before beginning employment.
• Excellent interpersonal, communication, clinical, and collaborative skills.
• Commitment to providing individualized, resident-centered care.

Responsibilities

• Provide individualized, one-on-one therapy services to residents.
• Lead and support the day-to-day operations of the rehabilitation program.
• Partner with the therapy team and community leadership to coordinate resident care.
• Support caseload development and identify opportunities to expand therapy services within the community.
• Help the team meet clinical, operational, and efficiency expectations.
• Build strong relationships with residents, families, team members, and community partners.
