About The Position

Relient Health is seeking an experienced Director of Rehabilitation (DOR) to lead a Senior Living Community in San Jose, CA. This is a full-time, direct-hire leadership opportunity for a seasoned therapy professional with proven experience in managing rehabilitation operations in a Senior Living Community setting. The ideal candidate thrives in leadership, enjoys mentoring therapy teams, and is passionate about achieving top-tier patient outcomes. This position is open only to licensed therapy professionals who already have experience as a Director of Rehab in a Senior Living Community setting. Clinicians looking to transition into their first management role will not be considered. The facility is a stable, mission-driven Senior Living Community known for quality therapy outcomes and a strong interdisciplinary team. The DOR will lead PT, OT, and SLP services, ensuring clinical excellence, efficient operations, and a supportive environment for both patients and staff.

Requirements

Active PT, PTA, OT, or COTA license in CA
Minimum 1 year of recent experience as a DOR in a Senior Living Community setting or equivalent (REQUIRED)
Strong communication and leadership skills
Knowledge of Medicare documentation, compliance, and billing standards

Responsibilities

Provide hands-on leadership to the rehab department and therapy staff
Ensure compliance with federal, state, and facility regulations
Oversee scheduling, staffing, documentation, and productivity goals
Mentor and evaluate team members to maximize performance
Collaborate with nursing, administration, and corporate teams on outcomes and goals
