About The Position

CommuniCare Rehab, a member of the CommuniCare Family of Companies, is hiring a full-time Director of Rehabilitation at Grande Pointe Healthcare Center. Therapists and leaders are in-house team members. The position provides an opportunity to make a direct impact on residents through therapy. CommuniCare is described as a rapidly growing company with 170 centers across IN, MD, MO, OH, PA, VA, and WV.

The Director of Rehabilitation performs and oversees therapy services, supports clinical programming and caseload management, collaborates with facility and corporate staff on denied therapy claims, and helps ensure accurate billing and documentation. The role also includes training and educating team members, patients, residents, and families as appropriate.

Requirements

• State-licensed clinician (PT, OT, SLP, PTA, or COTA) from an accredited program.
• Excellent oral and written communication skills.
• Effective leadership skills.

Responsibilities

• Perform evaluations, screens, and treatments for facility patients and residents who need therapy services.
• When serving as the evaluating therapist, evaluate patients or residents under physician order and develop treatment plans according to their needs.
• Implement treatment plans through direct treatment and education of treatment staff, including therapists and other team members involved in the plan.
• Attend or lead interdisciplinary meetings as appropriate.
• Serve as a resource for clinical programming and caseload matters.
• Relay clinical findings and make recommendations to other members of the rehabilitation team.
• Collaborate with the facility and corporate Denials Management Specialist regarding ADRs or denied therapy claims.
• Conduct staff and family training and education through formal and informal in-services and consultations when appropriate.
• Maintain accurate daily billing information according to facility procedures, train or retrain team members, and comply with rehabilitation department documentation requirements.
