About The Position

The Director of Palliative and Primary Care is an Advanced Practice Registered Nurse Practitioner (APRN) with prescriptive authority, who develops and manages the Arkansas Palliative and Primary Care programs. The Director of Palliative and Primary Care may also, in collaboration with the Chief Medical Officer (CMO) and other physicians, provide direct patient care in a variety of settings (i.e., private home, nursing home, hospital). This position will be 50% administrative and 50% clinical.

Requirements

Bachelor’s Degree in Nursing
Advanced Practice Registered Nurse Practitioner licensed in Arkansas; prescriptive authority required.
Minimum of two years of RN or APRN palliative care experience preferred.
Minimum of two years administrative experience preferred.
Valid Arkansas driver’s license
Personal automobile insurance

Knowledge of palliative and primary care with emphasis on symptom control associated with terminal illness.
Knowledge of the holistic approach toward patient care and the nursing process as it applies to hospice patients and families.
Knowledge of hospice certification/recertification eligibility requirements.
Knowledge and acceptance of hospice philosophy and principles of care.
Knowledge of computer software applications.
Skill in organizing and prioritizing workloads to meet deadlines.
Ability to communicate effectively both orally and in writing with co-workers and other customers.
Ability to follow basic safety policies and procedures.
Ability to use good judgment and to maintain confidentiality of information.
Ability to work as a team player.

Must be able to work in a constant state of alertness and concentrate for long periods of time while performing life-threatening procedures, monitoring and/or operating equipment used to perform medical procedures, and/or working with controlled substances and/or medicine.

Responsibilities

Leads, develops and coordinates all palliative and primary care department initiatives and programs.
Performs all supervisory responsibilities for non-physician members of the palliative and primary care staff.
Develop, align, and mobilize members to successfully achieve clinical, operational, and strategic priorities for the organization.
Establish program structures for accountability with scheduled staff assignment oversight in all locations.
Completes performance evaluations for all assigned staff in a timely manner.
Assesses and makes necessary updates to the palliative and primary care staff organization to best benefit patient care.
Updates all palliative and primary care staff job descriptions as needed.
Meets regularly with palliative and primary care staff to create departmental policies, procedures, and protocols, to educate on current trends and clinical knowledge, and to discuss operational challenges.
Maintains and updates all clinical forms related to palliative and primary care.
Manages departmental FTE’s within established guidelines including controlling agency and overtime expenses.
Identifies and develops advancement/succession planning for hiring, training and retention of staff.
Responsible for the identification and realization of program opportunities through reduction of variation in cost, quality, and resource utilization by working collaboratively with all contracted organizations to ensure effective communication.
In cooperation with the clinical and administrative management team, develops, recommends, and implements required palliative and primary care policies and procedures.
Oversees timely completion of all data reporting and maintenance of database.
Develops departmental budget and manages expenses within established limits.
Ensures compliance with all applicable state and federal laws, regulatory standards, and organizational policies and procedures.
Ensures compliance with the Center for Medicare Services (CMS) in relation to billing, and payment for all palliative and primary care visits.
Ensures compliance with all contractual quality, performance and/or risk-based measures.
Reviews, codes (when necessary), and authorizes all palliative and primary care related invoices.
Participates in QAPI activities and other committees as assigned.
Actively supports and cooperates with other departments/disciplines in order to achieve organizational goals.
Collaborates with HIM to track and monitor MACRA to ensure that program stays compliant.
Utilizes approved clinical guidelines and evidence-based protocols when providing care.
Performs comprehensive and focused evaluation of patients.
Provides services related to health maintenance and promotion, palliative and primary care and end of life care.
Recommends laboratory and radiology tests as appropriate, in collaboration with the palliative care and/or physician.
Collaborates with home health nurse, hospital nurse, attending and palliative care physicians and others as needed to discuss identified patient needs and to order medication adjustments, treatments, and other care.
Performs other therapeutic or corrective measures as indicated, including urgent care as necessary.
Consults with Chief Medical Officer or designee as needed, informs primary physician of services provided and collaborates with other physicians as needed; provides written reports to primary physician upon request.
Prepares and maintains accurate patient records, charts and documents to support sound medical practice.
Provides training for staff, medical residents and students as needed.
Adheres to the practice of confidentially regarding patients, families, staff and the organization.
Maintains current knowledge and skills in the Advanced Practice Registered Nurse Practitioner category.
Utilizes and models excellent customer service skills at all times; seeks opportunities to assist patients, families and co-workers; demonstrates teamwork and cooperation.
Accepts direction and follows instructions from supervisor; seeks additional information as needed; works with minimal supervision.
Adheres to all organizational and departmental policies and procedures.
Continually meets organizational standards for attendance and punctuality; notifies supervisor in a timely manner when employee will be absent or late for work.
Attend conferences and educational offerings to expand knowledge of practices and principles of palliative care.
Attends all required meetings and in-services; seeks opportunities for additional professional development activities as appropriate.
Performs clinical duties in any area that Arkansas Palliative and Primary Care, LLC is contracted.
Performs other duties as assigned.
