About The Position

The Director of Health and Wellness is responsible for overseeing the health and well-being of residents. This role involves conducting health evaluations, monitoring resident health status, supervising staff, ensuring compliance with regulations, and fostering a positive environment. The Director collaborates with the Executive Director and other stakeholders to implement quality improvement initiatives and manage various operational aspects of the community.

Requirements

  • Must have a thorough understanding of health evaluations for new residents.
  • Must be able to monitor resident health status through evaluations and care conferences.
  • Must be able to monitor resident evaluations and service plans for team compliance.
  • Must be able to react immediately to suspected resident abuse.
  • Must have experience supervising personnel, including hiring, coaching, orientation, scheduling, and training.
  • Must be able to provide team member evaluations.
  • Must be able to perform disciplinary actions.
  • Must be able to attend management and staff meetings.
  • Must be able to work with Executive Director and The Support Center to conduct community audits.
  • Must be able to review incident reports and conduct investigations.
  • Must be able to work with Executive Director to ensure Continuous Quality Improvement (CQI).
  • Must be able to support Executive Director in maintaining compliance with regulations.
  • Must ensure employees work in a safe manner.
  • Must foster teamwork and show initiative.

Nice To Haves

  • Experience with HIPAA, OSHA, and State Survey issues.
  • Familiarity with Safety Committee Standards.
  • Knowledge of procedures for treatment, investigation, return to work, and management of injured workers.
  • Communication with care providers and management companies.

Responsibilities

  • Performs thorough health evaluation on all new residents upon move-in.
  • Monitors the health status of current residents through evaluations and care conferences.
  • Monitors resident evaluations and service plans for team compliance, including medication side effects, programming, meals, complaints, cognition, behavior, ADLs, and special requests.
  • Reads daily alert charting and initiates actions to improve resident health status.
  • Coordinates and attends resident care conferences and service plan reviews.
  • Reacts immediately to suspected resident abuse, conducting investigations and notifying appropriate parties.
  • Supervises all Assisted Living and Memory Support personnel, including hiring, coaching, orientation, scheduling, and training.
  • Provides team member evaluations and communicates employee relations issues to the Executive Director.
  • Performs necessary verbal and written disciplinary actions for supervised staff.
  • Attends daily management meetings, staff meetings, and other delegated meetings.
  • Works with the Executive Director and The Support Center to conduct community audits.
  • Reviews incident reports, conducts investigations, and plans corrective actions.
  • Works with the Executive Director to ensure Continuous Quality Improvement (CQI) activity.
  • Works with the Executive Director to identify and set priorities to resolve service-related issues and improve quality.
  • Supports the Executive Director in maintaining compliance with all local, state, federal, and licensing rules and regulations.
  • Ensures all employees work in a safe manner and that unsafe actions are managed.
  • Fosters teamwork, shows initiative, assists others, and has a positive impact on co-workers, residents, and visitors.
  • Performs other duties as assigned or needed.
