About The Position

Amidst clinical workforce shortages, rising healthcare costs, and ongoing concerns about inequity, IMPaCT unlocks the power of the grassroots community health workforce. IMPaCT Care finds, trains, and enables Community Health Workers (CHWs): caring people who keep their communities healthy. IMPaCT has built the largest and most scientifically-proven CHW platform in the country, used across 22 states by over 70 organizations. IMPaCT has achieved significant outcomes, including annual savings per patient, improved mental health, and a high Net Promoter Score. This role is offered through a partnership between Highmark Health in Pittsburgh, PA, and IMPaCT Care. While the role supports Highmark Health's community health initiatives, the position is directly employed by IMPaCT Care.

Requirements

  • Graduate degree in Social Work, Public Health, Population Health, or Healthcare Administration.
  • 5 years of healthcare experience with 2+ years in a leadership role with responsibility for operational excellence.
  • Track record of developing and leading strategic initiatives.
  • Track record of achieving outcomes through program implementation.
  • Experience managing budgets.
  • Experience with recruiting, managing, and developing high-performing staff.
  • Experience with developing community-facing programs, including building and sustaining successful external partnerships.
  • Exceptional presentation, interpersonal, and writing skills.
  • Applicants must be U.S. citizens.

Responsibilities

  • Oversee a team structure of one Manager and 10 CHWs, providing leadership, coaching, and accountability at both levels.
  • Build the team by partnering with community-based organizations to recruit and hire caring, trustworthy CHWs and administrative staff.
  • Work on-site to oversee day-to-day programmatic and personnel operations, supervising staff in alignment with program goals.
  • Integrate CHWs into community and clinical settings and ensure high-quality patient support.
  • Deliver and report high performance on quality outcomes and program satisfaction for all individuals receiving CHW support.
  • Achieve high patient and staff satisfaction to promote retention and growth of the program.
  • Build IMPaCT's presence in the Pittsburgh area through partnerships and strategic selection of engagement activities.
  • Ensure high-quality delivery of client services — including strategic planning, hiring, training, professional development, and programmatic support — to support Highmark Health's CHW program locally.
