Requirements

  • Active Registered Nurse (RN) license in the state of New Jersey.
  • Valid driver's license, reliable transportation, and current auto insurance.
  • Minimum of two (2) years of nursing experience in hospice, home health, or community health.
  • Minimum of one (1) year of supervisory or leadership experience.
  • Strong knowledge of hospice regulations, end-of-life care, symptom management, and interdisciplinary care planning.
  • Excellent communication, leadership, organizational, and problem-solving skills.
  • Ability to effectively support staff while providing exceptional service to patients and families.

Nice To Haves

  • Previous hospice leadership or management experience.
  • Experience overseeing interdisciplinary clinical teams.
  • Knowledge of quality assurance and regulatory compliance programs.

Responsibilities

  • Provide leadership and oversight of all direct and indirect patient care services delivered by the interdisciplinary team.
  • Collaborate with the Clinical Team Coordinator to ensure high-quality patient care and positive patient outcomes.
  • Lead interdisciplinary team (IDT) meetings, facilitating care plan discussions, problem-solving, and team collaboration.
  • Recruit, hire, supervise, mentor, and evaluate clinical team members.
  • Ensure timely, accurate, and compliant clinical documentation.
  • Monitor compliance with federal, state, Medicare, accreditation, and agency standards.
  • Partner with the Compliance Department to identify educational needs and implement staff training initiatives.
  • Conduct regular visits to contracted facilities to maintain strong relationships and provide education and support.
  • Monitor operational expenses, including payroll, supply costs, mileage reimbursement, and budget-related expenditures.
  • Support quality improvement initiatives and promote a culture of clinical excellence.
