The Director of Case Management (DCM) for Visiting Nurse Health System’s (VNHS) Elderly and Disabled Waiver Program (EDWP) ensures high-quality, member-centered care coordination for older adults and individuals with disabilities who have medical and social needs. Serving as a liaison to the Executive Director of Operations (EDO), the DCM is a key member of the leadership team, aligning case management operations with organizational goals and promoting equitable access to care. The DCM leads a large team, including the Sr. Manager of Case Management, Team Leads, and clinical and non-clinical staff across multiple locations. The DCM develops and implements case management models to promote service access, improve health and social outcomes, and ensure compliance with regulatory, contractual, and funding requirements. The DCM oversees multidisciplinary teams, sets performance standards, and builds partnerships with healthcare providers, social service agencies, government entities, and community organizations. WHO THRIVES IN THIS ROLE This position rewards someone who thinks in systems, not just tasks — building the case management models, performance standards, and policies that then govern outcomes across a large, multi-location team, rather than personally managing each person in it.
